We will ask you fill out a detailed Pain form (PDF) and a Patient History form (PDF). The topics covered in these forms include :

Medications
Illnesses
Familial illnesses
Accidents
Surgeries
Previous treatments
How your present condition affects your life

You may download the forms using the links provided above or we can fax or email them to you at your request.

History Review & Consultation

We will have follow-up questions regarding your health status and any specific issues that affect your health. This is the time to define and discuss your treatment goals.

Examination

We will perform a general health screening examination as well as one specifically directed to the region or area of your stated problem.

Treatment

Based on the findings from your paperwork, consultation, history, and exam, you will receive a treatment at that time on that same day.

Second Visit & Follow-Up Treatment

Patients are usually seen within 1 to 5 business days for a follow-up appointment. This enables us to review the results of the first treatment and refine the treatment if necessary. A second treatment will be given at that time.
